Patient Dies After Brain X-Ray Meant for Another

PHILADELPHIA — A patient mistaken for another with the same last name died after he inadvertently had a brain X-ray at a Veterans Administration hospital, officials said Saturday. Doctors said they did not know if death was caused by the X-ray Thursday or if it was related to an allergic reaction to the dye used in the procedure.
